Can only make appt by faxing or dropping a note through the door. Who on earth has a fax machine and why would I go all the way to the dr office to drop a note through the door. Get an email address for goodness sake!

Please get email address, also applied for disability have epilepsy, my Psychiatrist told me not to work and to get on Disability. I gave Marquis the form, once I got denied for disability I went and looked. Basically he told them that it doesn’t effect my daily living activities, I have temporal lobe epilepsy, I can’t work right now and I’m on new meds! I know kids who have ADHD who have disability and peoples with depression too. Everytime I get a job I have seizures and get let go. I was gonna give my psychiatrist the papers but sadly he passed, he actually cared and saw me! Great man, but I never see and it’s hard to get In touch with marquis! Totally trashed the form tho!
