I highly recommend FORM Health Clinic in Nanaimo. The atmosphere is inviting , the reception always has a smile, and their online appointment system makes booking a breeze. If you're looking for a massage, John Chahley is an amazing RMT. His techniques may not feel pleasant but it's like 10 massages in one!! One happy customer!

This well run rehabilitation clinic provides a relaxing and safe atmosphere in which to convalesce. From the reception staff to the practitioners, everyone at the clinic is inviting and a pleasure to be around. Whether recovering from a tough injury or just wanting to work out some "kinks" Form is there every step of the way, providing excellent service and wonderful care. On January 14, I slipped on ice and fractured my left fibula, aka broke my ankle. I was in a no-touch cast and on crutches for six weeks. Once the orthopedic surgeon gave me the green light to take off the cast, I was left with a giant sprained ankle with lots of soft tissue damage, and it became obvious that there was also some injury to my knee. I was able to see Travis for a fibula adjustment, which helped a lot. However, there was still soft tissue pain in my knee. With social distancing in place, I spoke to Dr. Shook, and he recommended massage in two specific spots, as well as using an ice pack (20 min on/off). His advice has definitely helped speed up the healing in my knee. I am walking without almost no pain at all now. Thank you!

Dr. Shook is wonderful. First visit was a complete check. He explained that my shin pain was actually from my back. Could mot sleep on either side due to my hips. I went a couple times a week then longer between as I was doing so well. Highly recommend him as well as the front desk ladies. Always so nice to deal with.

I really recommend the Form Clinic. My first experience; a friendly conversation over the phone with the receptionist setting up appointment, no parking issues upon arrival, greeted with a smile and down to business with the receptionist. Thereafter, the professional workers I've had contact with were very patient with me and helpful. Booking back to back appointments does not seem to be a problem. For example, I can go from Chiropractic treatment to a Massage in one visit. Thank you Form Clinic for bringing relief to my aches and pains. Special kudos to Lora, Karin and Terry. Thank you.
