I have had trouble booking here and my first appointment, they don't call back or email back promptly, my acupuncturists was 10mins late on my first appointment where I only received 10 mins of acupuncture for a very expensive and short initial appointment. I booked in for an RMT and there was no call back for an ICBC inquiry and I wasn't even booked in at all, and I was coming form Quadra Island, good thing I called in to inquire.The receptionist offered no solutions. This place is nice on the outside but to me Form Wellness seems over priced and has poor customer service.

Amazing experience! The space is beautiful. Laura did a very thorough consultation with me and explained what she was doing. I loved the added touch of the warm massage table when I got in. I would highly recommend!

I highly recommend Form, having seen both RMTs and Acupuncturist for a variety of reasons. Every time the clinic is running on time, incredibly welcoming and comfortable and I walk out not only feeling better but with a greater understanding of my body and how to take care of it. The only downside is getting appointments as they are so good they book up fast!

Have been attending Form for RMT services for over a year. I initially booked using their online appointment service, which was easy to navigate and very convenient. I have seen Laura (RMT) for all of my appointments and have been impressed by her knowledge and professionalism, particularly within the context of pandemic health care delivery. Every appointment has had a positive impact on my well being. The clinic is perfectly clean and all staff members have been pleasant, welcoming and professional. The Form team has been prompt in their responsiveness regarding ever changing pandemic requirements to maintain service availability during this challenging time. This is one place that I go to get some relief from back pain. Staff here are very knowledgeable, take their time to understand your pain and treat accordingly. They would also follow up later to see how the treatment worked and take feedback. During your next visit they would adjust the treatment based on your feedback to get better relief from the pain which works amazingly. Clean and nice facility, Staff are welcoming and Easy booking methods. Overall a great experience every time I visit this facility.

I've been a client at Form for just over a year, exclusively seeing Laura. It's a true testament to how well a business is doing when they are constantly booked up! Laura asks at the beginning of every appointment how I have been doing and goes into finer details, such as how my work station is arranged, what activities I have been doing, etc. to better determine how my treatment would be best approached, and then goes about her business like a true professional. I find myself leaving my massages "sore", but that good type of sore! There is a reason that I continue to come back...and Laura aside I've heard nothing but amazing things from the entire team at Form! I'll admit after moving back to Campbell I tried a few places and while they have provided good results, Laura has consistently made me feel like a million bucks. I absolutely recommend this facility for all the services they provide! Once COVID settles down I'm looking to start yoga there as well to continue to better balance my life.

Merely warm compresses and ROM is not called a complete massage!!! I took massage just for a change but wasn't a good experience at all.The front desk lady didn't even ask me about how long do i need the service while booking,instead she said it's mandatory to have one hour massage if you are a new customer when i requested for less time.

Fantastic clinic. I have had massages from Laura Chomyshyn for a few years now and had chronic back pain prior. She figured out what was wrong when many other RMTs could not. Great clinic, great vibes. Great Zeus doggo friend makes it even better. Would recommend to anyone.
