though i would recommend adding "issued date" on top of the test date as I almost got screwed over going through covid customs in korea as it wasn't originally a part of their form, the reason being it being a rapid test which results come out only 20 minutes after the test. however entry requirements for korea at the time specified an issued date for the form so I asked it to be added and they obliged without a fuss. They actively looked for the issued date at customs and I had to point out that it's in the note section in small letters.
